Iran also had moments of danger and thought they had taken the lead through Mehdi Taremi, but the goal was ruled out for offside after a VAR review.
The match remained tightly contested with chances at both ends.
The game turned in the 66th minute when Belgian defender Nathan Ngoy received a straight red card for denying Taremi a clear goalscoring opportunity.
Despite playing against 10 men for the final stages, Iran could not find the breakthrough goal.
The draw leaves both Belgium and Iran on two points in Group G, making their final group matches decisive.
